Case Manager - Part Time

  • M-F 12:00 PM - 4:00 PM - 24hrs/ Week

  • Assist with COVID contact tracing and data management

  • Process incoming reports of potential COVID-19 cases/exposures and ensure completion of data.

  • Follow up to obtain additional relevant information. Create and maintain case records.

  • Act as a resource for interjurisdictional communication and transfer of case and and contact information. Verify established quarantine and isolation recommendations, document and communication same to case manager, and escalate complex cases to program lead.

  • Facilitate reassessment process flow of cases that evolve based on changes in illness, contacts, or test results.

  • Evaluate employee personal travel for applicable government restrictions and determine appropriate quarantine actions.

  • Support data integrity and analysis as needed.

  • "Excellent and tactful interpersonal skills that allow them to build and maintain trust with company contacts.

  • Understanding of and ability to explain in plain language, the medical terms associated with COVID19 case management such as the principles of exposure, infection, infectious period, potentially infectious interactions, symptoms of disease, presymptomatic and asymptomatic infection, types of tests used to diagnose infection, and available nonpharmaceutical interventions (e.g., isolation/quarantine, social distancing, environmental surface cleaning).

  • A keen understanding of the need for information confidentiality. Resourcefulness in locating and communicating with contacts who may be difficult to reach or reluctant to engage. Use of protocols that include referral of highrisk and complex cases to chain of command. Critical thinking, Problem solving, Data entry, Administrative support, Ethical and professional conduct, Cultural humility and competency, Active listening, Open communication, Emotional intelligence, Flexibility and adaptability, Detail oriented."

  • Required Certification/Education - 4 year degree
